I have commented on this before For years the show has downplayed the fact
that Julie and Hope are sisters. For literally several years, the show
never acknowledged they have this relationship and instead always tried to
emphasize that Julie is Hope's step mother. Therefore, in the show's
mantra, Ciara thinks of Julie as her step-grandmother, even though you are
absolutely correct she is also her aunt, which is really the more logical
relationship since it shows they are blood relatives.

As to why the show tries so hard to minimize the fact that Julie and Hope
are siblings, the most popular theory is that Kristian Alfonso doesn't want
anyone to think of her as "old", so she wants to make sure it is clear in
everyone's mind that she is a lot younger than Julie. Therefore, so the
theory goes, Kristian has asked the writers to make sure they avoid too many
direct references to her and Julie being sisters and to emphasize that she
is Julie's step-daughter.

I have heard Hope called grandmother.

Right, Claire has started calling her that the last couple of years. But
when Claire was first on the show, you almost never heard her say it and it
was almost like Claire and Hope weren't related. For a long time, the fact
that Claire and Ciara are related (Ciara is Claire's aunt) was also never
mentioned.

But that has started changing a bit.
